All-on-5 Implants

All-on-5 implants are a full-arch restoration technique where five dental implants are placed in one jaw to support a fixed bridge of replacement teeth. This treatment is usually considered when you have lost most or all teeth in an upper or lower jaw, or when existing teeth cannot be saved due to decay, gum disease or fractures. The five implants act as artificial tooth roots, anchoring a bridge that replaces a complete row of teeth in a more stable way than removable dentures.

On this page you will learn what All-on-5 implants are, who may be a suitable candidate, how the procedure is planned and carried out, and what to expect during healing and long-term care. You will also read about possible risks, expected treatment times, eating after surgery and how treatment can be organised if you are travelling to Antalya from abroad.

What are All-on-5 implants and how do they work?

All-on-5 implants are a dental implant solution where five titanium or titanium-alloy posts are placed in one jaw to support a fixed full-arch bridge of artificial teeth. They are designed for people who have many missing teeth or severely damaged teeth and want a stable alternative to removable full dentures.

In an All-on-5 system, each implant is inserted into the jawbone and acts like an artificial tooth root. After the bone heals and fuses to the implant surface (a process called osseointegration), the implants become firmly anchored. A custom-made bridge, usually consisting of 10–14 teeth for one arch, is then attached to these implants using small screws. You cannot remove this bridge at home; it is fixed and can only be taken off by the dentist if needed for maintenance.

The five implants are usually distributed strategically along the jaw. Two or three are placed in the front where the bone is often thicker and denser, and the others may be angled slightly in the premolar or molar areas to maximise contact with the available bone. This positioning spreads chewing forces across the arch while using fewer implants than placing an implant for every missing tooth.

All-on-5 implants work by:

  • Restoring chewing function so you can bite and chew more comfortably compared with loose dentures.
  • Improving stability, because the bridge is anchored into the bone rather than resting on the gums.
  • Helping maintain bone, as implants can reduce the rate of bone loss that usually continues after teeth are removed.
  • Supporting facial structure, which may help your lips and cheeks look more supported compared with not wearing dentures.

All-on-5 is one of several full-arch implant concepts. The exact number and position of implants (for example All-on-4, All-on-5 or All-on-6) is chosen after detailed assessment of your bone quantity, bone quality, bite and general health. A dentist specialising in implant dentistry will recommend the configuration that matches your situation, expectations and budget.

Who is a good candidate for All-on-5 implants?

A good candidate for All-on-5 implants is usually someone who is missing most or all teeth in one jaw, has enough bone to support five implants, and is in generally stable health. A detailed examination and imaging are always needed before confirming suitability.

You may be considered for All-on-5 treatment if some of the following apply to you:

  • You are already using a full denture that feels loose, unstable or uncomfortable.
  • You have many teeth that are broken, decayed, infected or mobile and are no longer predictable to restore individually.
  • Your gums are currently free of active, untreated gum infection, or such infection can be treated before implants are placed.
  • You have enough jawbone height and width, or can achieve it with bone grafting if the dentist advises this.
  • You do not smoke, or you are willing to reduce and ideally stop smoking before and after surgery to improve healing.
  • Your diabetes, blood pressure or other systemic conditions are well controlled under the care of your physician.
  • You understand that implants require long-term cleaning and regular check-ups, and you feel ready to follow this routine.

Age by itself is not a strict barrier. Many older adults successfully receive full-arch implants if their general health and bone condition are suitable. The key questions are: can you safely undergo oral surgery, and will the bone be able to heal around the implants?

During assessment, the dentist will check:

  • Your medical history, medications and any allergies.
  • The condition of remaining teeth and gums.
  • Your bite and jaw movements.
  • X-rays and 3D scans to evaluate bone density and anatomy.

For example, if you have several painful, mobile teeth and a denture that keeps slipping, All-on-5 implants might offer a way to remove failing teeth, place implants and fit a fixed bridge so you can talk and eat more confidently. If your bone is thin in certain areas, the dentist may adjust implant positions or consider another full-arch concept such as All-on-4 treatment or adding extra implants for more support.

Everything You Need to Know

More about All-on-5 Implants

When are All-on-5 implants not suitable or delayed?

All-on-5 implants are not suitable or may be delayed when your general health, bone condition or oral status would make surgery too risky or healing too uncertain. The decision is always individual and made after a full examination and discussion with both the dentist and, when necessary, your medical doctor.

Situations where All-on-5 may be postponed or avoided include:

  • Uncontrolled systemic diseases such as poorly controlled diabetes, severe heart disease, recent stroke or immune disorders that are not under medical supervision.
  • Active gum disease (periodontitis, an inflammation of the supporting tissues of the teeth) that has not yet been treated. Gum infection must usually be stabilised first.
  • Insufficient bone volume where even angled implants or grafting would not provide predictable support, or where important structures such as nerves or the sinus are too close.
  • Ongoing chemotherapy or head and neck radiotherapy, which can affect bone healing. Implant surgery is often delayed until your oncologist confirms it is safe.
  • Heavy smoking or vaping that you are not willing to reduce. Nicotine can impair blood flow and healing, and it significantly increases the risk of complications like implant failure.
  • Untreated severe teeth grinding (bruxism) and clenching, which may put extreme forces on new implants. These conditions do not always rule out treatment, but they often require protective measures and careful design of the bite.
  • Pregnancy, particularly in the first and third trimesters. Elective implant surgery is usually postponed until after birth to avoid unnecessary stress and exposure to diagnostic radiation.
  • Substance abuse or conditions that make it difficult to maintain daily oral hygiene and attend follow-up appointments.

There are also times when implants are not ruled out, but timing is important. For example, if teeth have just been extracted, some dentists prefer to place implants immediately in the same visit, while others recommend waiting a few weeks or months for initial healing, depending on the infection level and bone quality. If you are on blood thinners, the dentist may consult your physician to see if adjustments are needed, or plan the surgery in a way that minimises bleeding risks.

Delaying treatment can sometimes be beneficial. It allows time to improve oral hygiene, stabilise medical conditions, stop smoking or complete gum therapy so that the implant environment is healthier. Your dentist will explain the reasons clearly and suggest temporary solutions such as dentures until you are ready for full-arch implants.

How are All-on-5 implants planned with X-rays and 3D scans?

All-on-5 implants are planned using detailed X-rays and 3D scans to evaluate your bone, nerves, sinuses and overall jaw anatomy so implants can be placed in safe, stable positions. Imaging is essential; without it, the dentist cannot reliably design your treatment.

Planning usually includes the following steps:

  • Panoramic X-ray: This wide view shows both jaws, sinuses and important nerves. It gives a first impression of bone height, remaining roots, infections and any cysts or impacted teeth.
  • 3D cone beam CT scan (CBCT): This scan creates a three-dimensional model of your jaws. The dentist can view cross-sections at any point, measure bone thickness and height precisely, and see the exact position of nerves and sinus cavities.
  • Clinical examination: Imaging is combined with a hands-on exam of your gums, bite, facial profile and jaw movements.

Digital planning software is often used with CBCT data. The dentist can:

  • Virtually place five implants on the 3D model to check angles and depths.
  • Plan how to avoid nerves in the lower jaw and sinus spaces in the upper jaw.
  • Estimate where the bridge teeth will emerge from the gums, which affects both appearance and cleanability.
  • Assess whether bone grafting or sinus lifting might be needed in some areas.

In many cases, this digital plan is then used to create a surgical guide, a custom template that fits onto your gums or remaining teeth. During surgery, the guide helps position the drills and implants in the exact places that were pre-planned on the computer, which can improve accuracy and predictability.

The planning stage is also when the design of your future bridge is discussed: approximate tooth shape, size, and how much gum-coloured material may be needed to support your lips and cheeks. Photographs and sometimes models of your existing dentures can help the dental technician design a full-arch bridge that suits your face and speech patterns. Nothing is final at this stage; the initial design can be adjusted during try-ins and follow-up visits as you share feedback about comfort and appearance.

How is the All-on-5 implant procedure done step by step?

The All-on-5 implant procedure is usually done in several stages, starting with planning and preparation and ending with the fitting of your definitive full-arch bridge. Details vary from person to person, but the general sequence is similar.

A typical step-by-step process looks like this:

  1. Assessment and planning
    • Medical and dental history review, clinical examination and imaging (X-rays and 3D scans).
    • Discussion of your expectations, possible alternatives and limitations.
    • Digital treatment planning and, if used, preparation of a surgical guide.
  2. Pre-surgical preparation
    • Professional cleaning and treatment of any active gum infections.
    • Extraction of teeth that cannot be saved, either in the same appointment as implant placement or earlier, depending on your case.
    • Impressions, scans and records for making a temporary bridge or denture.
  3. Implant surgery
    • Local anaesthesia is given so you do not feel pain during the procedure. Sedation may be considered for very anxious patients if appropriate.
    • The dentist exposes the jawbone through small incisions in the gum or with flapless techniques when conditions allow.
    • Five implant sites are prepared according to the digital plan, sometimes with angled positioning in the back areas.
    • Five implants are placed into the bone, and their stability is measured.
    • If needed, minor bone grafting or contouring is performed around the implants.
    • Gums are stitched around the implants.
  4. Temporary restoration
    • In many cases, a temporary fixed bridge or an adjusted denture is attached on the same day or within a short time, as long as the implants are stable enough.
    • If immediate loading is not safe, you may use a removable denture during the initial healing period without biting hard on the implant area.
  5. Healing and follow-up
    • The bone heals around the implants over several weeks to months.
    • You attend check-ups so the dentist can monitor healing, remove stitches if needed, and adjust your bite.
  6. Final bridge
    • After osseointegration, final impressions or scans are taken.
    • A custom full-arch bridge is fabricated, often with a metal substructure and tooth-coloured materials.
    • The final bridge is screwed onto the five implants, and your bite, speech and aesthetics are carefully checked.

At the end of treatment, you receive cleaning instructions and a follow-up schedule. The dentist will explain how often to return for professional checks and what signs to watch for, such as unusual looseness, pain or swelling around the implants.

How long does All-on-5 implant treatment take?

All-on-5 implant treatment can take from a few days for the initial surgical phase up to several months until the final bridge is delivered, depending on your bone condition, healing response and whether additional procedures like bone grafting are needed.

It helps to think of the total timeline in parts:

  • Initial consultation and planning: 1–2 visits over a few days or weeks. During this time, you have examinations, X-rays, 3D scans and treatment discussions.
  • Surgical visit: The implant placement appointment itself usually takes 2–4 hours per jaw. If extractions or grafting are needed, this may extend the duration.
  • Immediate temporary restoration: If your case allows placing a temporary fixed bridge on the same day, you may leave with attached temporary teeth within 24–72 hours. Some patients instead use a removable denture while the implants heal.
  • Healing and osseointegration: The bone generally needs around 3–6 months to fuse firmly to the implants. In some lower jaw cases with very good bone quality, the healing period can be shorter; in upper jaws or softer bone, more time may be needed.
  • Final bridge fabrication: Once healing is confirmed, making and fitting the definitive All-on-5 bridge typically takes several appointments over 1–4 weeks for impressions, try-ins and final placement.

Travel schedules for international patients are often arranged in two main visits. The first stay, usually 4–10 days, covers planning, extractions, implant placement and a provisional restoration. The second stay, after the healing period, often 5–10 days, focuses on final bridge fabrication and fitting. Exact timing is personalised and depends on your clinical situation and how far you need to travel.

The dentist will explain your specific timeline, taking into account your medical history, any medication changes that need coordination with your physician, and your ability to return for follow-up. Flexibility is sometimes necessary if healing is slower or if minor adjustments are needed to improve comfort or function.

Does the All-on-5 implant procedure hurt?

During the All-on-5 implant procedure you should not feel pain because local anaesthesia is used to numb the area, but it is normal to feel pressure and vibrations. Afterwards, some soreness, swelling and mild to moderate discomfort are expected for a few days as part of normal healing.

Before surgery starts, the dentist checks that the whole treatment area is fully numb. If you are anxious, you can discuss options such as sedation, if appropriate for your health. Throughout the procedure, you may feel pushing, pulling or drilling sensations, but sharp pain should not occur. If you feel uncomfortable, it is important to signal this so that more anaesthetic can be given.

After the numbness wears off, most people experience:

  • Soreness around the implant sites and gums.
  • Swelling and sometimes bruising of the cheeks or lips, which often peak 2–3 days after surgery.
  • Mild oozing or small spots of blood from the surgical area on the first day.
  • Stiffness when opening the mouth widely for a short period.

The dentist will usually recommend appropriate pain relief and, when needed, prescribe medication according to your general health. Following the aftercare instructions, such as using cold compresses on the outside of the face for short periods in the first 24–48 hours, keeping your head slightly elevated while resting, and sticking to soft foods, can make the first days more comfortable.

Everyone’s pain threshold is different. Many patients describe the experience as less uncomfortable than they expected, similar to or slightly more intense than having multiple extractions. If you have severe pain that does not improve with the recommended pain relief, or if the pain suddenly gets worse after initially calming down, you should contact the clinic promptly for assessment, as this can be a sign of infection or other complications.

What is recovery like after All-on-5 implants?

Recovery after All-on-5 implants usually involves a few days of acute healing discomfort, followed by several weeks of gradual improvement as the gums settle and the bone fuses with the implants. Most people can return to light daily activities within a few days, while full bone integration takes several months.

In the first 24–72 hours you can expect:

  • Swelling and some bruising around the cheeks and jaw area.
  • Mild bleeding or pink saliva, usually controlled by gentle pressure with gauze as instructed.
  • Soreness when swallowing or moving the lips and cheeks.
  • A preference for very soft, cool or lukewarm foods.

Common advice during this time includes:

  • Applying cold compresses on the outside of the face at intervals to help limit swelling.
  • Avoiding hot drinks and foods until sensation fully returns, to prevent burns.
  • Not spitting forcefully or using straws, as suction can disturb early blood clots.
  • Resting and avoiding heavy exercise for a few days.

In the first 1–2 weeks, stitches (if non-dissolving) may be removed, and the gums begin to look and feel more normal. Pain usually decreases significantly, and any bruising starts to fade. You can usually increase activity levels, but chewing on the implants should still be gentle, following your dentist’s dietary instructions.

From weeks to months, recovery focuses on bone healing and getting used to the new teeth:

  • Osseointegration continues quietly beneath the gums; you will not feel this process.
  • You attend check-ups to check the stability of the implants and adjustment of the temporary bridge if needed.
  • You progressively adapt your speech and chewing. Some words may feel unusual at first, but most people adjust with practice.
  • Oral hygiene routines around the bridge and implant sites are refined, often using special brushes or flossing aids.

Redness, persistent swelling, warmth, bad taste, pus, or pain that worsens instead of improving are not typical and should be reported promptly. Likewise, if the temporary bridge feels unusually loose, breaks, or causes sore spots, contacting the dentist helps prevent more serious problems. Regular follow-up supports a smoother recovery and protects the health of your implants and surrounding tissues.

What can I eat after All-on-5 implant treatment?

After All-on-5 implant treatment you should start with very soft foods and gradually progress to firmer textures only as healing allows and your dentist confirms it is safe. Biting hard or chewy foods too early can overload the implants and affect healing, especially in the first weeks.

A common staged approach is:

  • First 48–72 hours
    • Choose cool or slightly warm, smooth foods such as yoghurt, blended soups (not too hot), mashed potatoes, smoothies eaten with a spoon, and well-mashed bananas.
    • Avoid hot, spicy, crunchy or seedy foods that can irritate the surgical area.
    • Do not chew directly on the implant sites; let foods melt or swallow them with minimal chewing.
  • First 1–2 weeks
    • Introduce soft foods that require a little chewing, such as scrambled eggs, soft pasta, well-cooked vegetables, soft fish or finely minced meat, and cottage cheese.
    • Still avoid hard crusts, nuts, chips, raw carrots, tough meat and sticky sweets, as these can stress the implants or get stuck under the bridge.
  • After the initial healing phase
    • Gradually move to a “soft-chew” diet as recommended by the dentist, increasing variety but still avoiding extremely hard items like ice, hard sweets and bones.
    • Chew gently on both sides to distribute forces more evenly.

Even after you receive your final All-on-5 bridge, it is wise not to use your new teeth as tools to open packaging or bite very hard objects, as this can damage both the bridge and the implants. Cutting tougher foods into smaller pieces and chewing more slowly reduces unnecessary stress.

Good nutrition supports healing. Try to include a balance of proteins, soft fruits and vegetables, and adequate fluids. If eating is difficult at first, you can ask your dentist or physician about suitable nutritional supplements. Avoiding alcohol and limiting very sugary snacks can also help your mouth and overall health during recovery.

What are the risks and possible complications of All-on-5 implants?

All-on-5 implants, like any surgical and prosthetic dental treatment, carry risks and potential complications. Most procedures go smoothly when carefully planned, but it is important to understand possible problems so you can recognise early warning signs and follow advice to reduce risks.

Possible complications include:

  • Early implant failure: In some cases, an implant does not integrate with the bone and becomes loose. This can be due to infection, overloading, poor bone quality, smoking or other medical factors. A failed implant often needs to be removed and may be replaced after healing.
  • Infection: Bacteria can enter the surgical site, leading to increased pain, swelling, warmth and sometimes pus or fever. Good oral hygiene and following aftercare instructions reduce this risk, but it cannot be completely eliminated.
  • Bleeding and bruising: Some bleeding is normal at first, but heavy or prolonged bleeding is a concern, especially if you take blood-thinning medications. Bruising of the cheeks or under the eyes can occur but usually resolves on its own.
  • Nerve irritation: In the lower jaw, there is a sensory nerve that supplies feeling to the lower lip and chin. If an implant is too close, you may experience temporary or, rarely, long-term numbness or altered sensation.
  • Sinus-related complications: In the upper jaw, implants placed too close to or into the sinus can cause sinus symptoms. Careful 3D planning reduces this risk; sometimes sinus lifting procedures are done to create more bone.
  • Peri-implantitis: This is an inflammatory condition around implants, similar to gum disease, which can lead to bone loss over time. It is more common in smokers and those with poor plaque control. Regular check-ups and cleaning help detect and manage it early.
  • Mechanical issues with the bridge: Screws can loosen, teeth can chip, or the bridge can fracture under heavy forces. These problems usually require repair or adjustments at the clinic.
  • Bite and speech adaptation: Some people find their bite or pronunciation feels unusual at first. Most adapt, but sometimes small adjustments of the bridge are needed.

Serious complications such as severe spreading infection, trouble breathing or swallowing, or uncontrolled bleeding are rare but require immediate medical attention. If you develop strong pain that worsens, notable facial swelling, fever, difficulty opening your mouth, or bleeding that does not slow after applying gentle pressure, you should contact a dentist or seek emergency care promptly.

Comprehensive assessment, 3D planning, and transparent discussions about your medical history, smoking habits and expectations are important parts of reducing risks and supporting a stable long-term result.

How long do All-on-5 implants last and what affects success?

All-on-5 implants are designed as a long-term solution, and with proper planning, placement and care, they can function for many years. The lifespan of the implants and the bridge varies between individuals and depends on both biological and mechanical factors.

Several elements influence long-term success:

  • Bone quality and quantity: Denser, well-distributed bone tends to support implants more reliably. When bone is thin or soft, more careful planning, additional implants or grafting may be needed to distribute forces safely.
  • Oral hygiene: Daily cleaning around the bridge and implants is essential. Plaque build-up can lead to inflammation of the gums and bone loss around implants (peri-implantitis), increasing the risk of failure over time.
  • Smoking: Tobacco use is one of the strongest risk factors for implant complications. It affects blood flow, healing and the body’s response to plaque, and it can reduce the long-term success rate of full-arch implants.
  • General health: Well-controlled diabetes, stable cardiovascular health and a balanced immune system all support better outcomes. Uncontrolled systemic conditions can negatively affect bone and soft tissue health.
  • Bite forces and habits: Heavy grinding or clenching can overload the implants and bridge. Protective night guards and careful bite adjustment may be recommended if you have these habits.
  • Bridge materials and design: The strength and design of the bridge, including how it distributes biting forces and how easy it is to clean underneath, play a role in long-term comfort and durability.
  • Regular professional care: Routine check-ups and professional cleaning are important. They allow the dentist to detect small issues early, tighten screws if needed, and polish or repair worn areas before they cause bigger problems.

It is common for the underlying implants to last longer than the first bridge. Over time, the bridge may need repairs, relining, or full replacement due to wear, changes in the gums and bone, or aesthetic preferences. The dentist will discuss realistic expectations with you, including the possibility of maintenance procedures in the future.

Attending scheduled appointments, maintaining excellent home care and avoiding damaging habits such as biting on very hard objects directly with the bridge all support the longevity of your All-on-5 restoration. If you notice bleeding when cleaning around the bridge, persistent bad breath, new gaps under the bridge, or a change in how your bite feels, an early check-up can help protect your investment in your oral health.

How does getting All-on-5 implants in Antalya work for international patients?

For international patients, getting All-on-5 implants in Antalya usually involves remote pre-assessment, planned travel for the surgical phase, a healing period back home, and a second trip for the final bridge. Clear communication and careful scheduling help coordinate dental treatment with your flights, accommodation and time away from work or family.

The typical process includes:

  • Initial contact and information sharing
    • You usually start by sending recent panoramic X-rays, photos of your teeth and bite, and a short medical history.
    • The dental team assesses whether All-on-5 or another solution such as All-on-6 implants or dental bridges might be more appropriate.
    • A provisional plan, expected number of visits and an approximate timeline are explained, with the understanding that final decisions depend on in-person examination and 3D scans.
  • First visit to Antalya
    • On arrival, you have a full clinical examination, X-rays, 3D scans and treatment confirmation.
    • If suitable, extractions and implant placement are scheduled during this stay, often including fitting of a temporary fixed bridge or adjusted denture.
    • This first visit may last from several days to around two weeks, depending on the complexity of your case and your travel flexibility.
  • Healing period at home
    • You return home with detailed written instructions on care, diet and what symptoms to monitor.
    • You may also be advised to visit a local dentist for basic checks if you have any concerns during healing.
  • Second visit for final bridge
    • After the recommended healing period, you travel back to Antalya.
    • Final impressions or scans are taken, and you attend try-in appointments for your definitive All-on-5 bridge.
    • Once the bridge is fitted and adjusted, you receive final cleaning instructions and advice about future check-ups.

Planning your stays with some flexibility is wise in case extra healing time or adjustments are required. Travel insurance that covers health-related schedule changes can be helpful. Many patients combine dental treatment with some rest days in the city, as long as this does not interfere with immediate post-surgical care.

At Umut Dental Clinic Antalya, patients can share their details and X-rays for an online consultation before planning travel, and they are advised to maintain regular routine oral check-ups with a local dentist once All-on-5 treatment is complete.

The information on this page is for general education only and does not replace an examination, diagnosis or treatment. See a dentist for any concerns about your oral health.

I had dental treatment at Umut Clinic Antalya, and I can confidently say—three years later—that this clinic truly delivers on its promise of quality and guarantee. The results of my treatment have remained excellent over time, and I'm still very happy with everything they did.
The entire team is extremely friendly, professional, and caring. The dentists are highly trained and very experienced, which immediately gave me a sense of trust and security. They took time to explain everything clearly and made sure I felt comfortable throughout the process.
What also sets Umut Zahnklinik apart is their exceptional service for international patients. From airport pickup to accommodation support, everything was well-organized and stress-free. You really feel taken care of from start to finish.
I can highly recommend Umut Zahnklinik to anyone looking for high-quality dental care in Turkey. A great clinic with lasting results and a warm, welcoming team!
